HELLENIC LEAGUE Match Report Form - How to Complete
MRF from - You will select the team specific to your club, if you have more than one team in Hellenic Competition select the relevant team for the report you are completing.
Fixture Date - The date the fixture took place.
Actual KO Time - The actual time the kick off took place, should the KO be delayed for any reason the referee will report that. However you must include the time the game actually started.
League / Cup Fixture - The type of fixture/competition relevant to the match being reported on.
Attendance - For the home club only to put the correct figure in - if you are the away opponent add 0 - This feature is required for reserves and veterans games, home side to state any attendance if none add 0.
Home Team - Select the home team from the list, if the home team has more than one team playing in Hellenic Competition just select the name of the team.
Goals Scored - Goals registered for the Home Team
Away Team - Select the away team from the list, if the home team has more than one team playing in Hellenic Competition just select the name of the team.
Goals Scored - Goals registered for the Away Team.
Home Colours - List as: Shirts Colour Shorts Colour Socks Colour - details will be shown on your match sheet
Keepers Jersey - Colour of Jersey
Away Colours - List as: Shirts Colour Shorts Colour Socks Colour - details will be shown on your match sheet
Keepers Jersey - Colour of Jersey
Team Sheet - Applicable to your team only

Match Officials
Referee Surname First, then Christian/Given name
Assistant 1 Surname First, then Christian/Given name - if club official please state name as shown on match sheet
Assistant 2 Surname First, then Christian/Given name . if club official please state name as shown on match sheet
Fourth Official This will only apply to certain games if appointed. It is not an assessor.
Performance of Referee
Difficulty of Game - How the overall game was assessed by your team manager - Three options
a) Overall Decision Making - The general competence of decision making by the referee - Four options
b) Judgement of Major Decisions - The general competence of making match relevant decisions by the referee - Four options
c) General Control and Player Management - The general competence of running and controlling the game - Four options
Mark out of 100 - The mark accredited to the referee for his overall performance and control of the match,
Additional Comments If any area is marked ‘poor’ or the total mark is 60 or less, detailed, constructive, developmental comments are required. These comments must be constructive and relevant to specific instances.
Conversely if you feel the referee and or other officials should receive notification for a good performance, you may also add your comments.
Comments may be added with regard to other the officials performance, the name of whom the report refers to should be clearly shown.
MARKING OF REFEREE’S As Advised by Brain King (added 13th October 2007)
You will be aware this season that the marking of referee’s also requires the award of ‘Performance Indicator’s on three specific aspects, the grading being ‘OUTSTANDING’, ‘GOOD’, ‘AVERAGE’ & ‘POOR’. The Football Association has stipulated that the expected AVERAGE MARK is 70. Therefore the award of THREE PI’s of AVERAGE should attract a MARK OF 70, bearing in mind the degree of Difficulty of the Match.
Over the first few weeks of the season I have received many variations of marks & PI’s resulting in an unacceptable number being returned to clubs for re-assessment as the Mark did not match the PI’s awarded.
In view of this I have devised the following table to assist you in getting your awarded MARK to dovetail with the Performance Indicator’s using the AVERAGE MARK of 70 as the starter.
The marking range for POOR is a MAXIMUM of 65
Any MRF that includes POOR as a PI requires a written report on the need for development of the MATCH REFEREE on that specific PI.
The marking range for AVERAGE is 66 – 75
The marking Range for Good is 76 – 85.
The marking range for OUTSTANDING is 86+
A Performance MARK outside of the indicated range is not acceptable unless ONE, or More, of the PERFORMANCE INDICATOR’S is not indicated as:
EG – Average/ Average/ Average – Mark in range of 66 -75. The inclusion of ONE PI of POOR or GOOD will allow a -/+ of FIVE MARKS from the mid range.
EG - Average/ Average/GOOD = 75
The person awarding the mark should also take into account the degree of difficulty of the match, ‘EASY’, ‘NORMAL’, or ‘DIFFICULT. NORMAL being the MID-POINT
